Running SBS 2003 Premium SP1 (ISA 2004). The VS Admini Web Site
(http://computername.domain.local:1024/VirtualServer/VSWebApp.exe?view=1)
does not work reliably. Sometimes, I can't get in with a "Error Code 10061:
Connection Refused" from ISA. Sometimes, I get prompted to authenticate and
get in but it has loaded improperly (malformed/no images). Sometimes it
works OK.

How can I troubleshoot?

loading correctly is usually because ISA has blocked access to our
stylesheet file (VSStyles.css)

I had the exact same issue. enabling firewall logging show connection attempts on various interfaces (my box has multiple nics), suggesting that timeouts and/or DNS resolution may be an issue. For now, I have replaced the URL with one that is hardcoded to the relevant (main) IP of the SBS server, and Virtual Server 2003 R2 web interace no longer has a problem!

with http://192.168.16.2:1024/VirtualServer/VSWebApp.exe?view=1 and it works
